It aims to formalise the relationship between patients\, their general practice\, general practitioner (GP) and primary care teams. Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes. \n\n\n\nThe General Practice in Aged Care Incentive (GPACI) will make it easier for older people living in residential aged care homes to receive regular visits and care planning services from their responsible GP and practice. \n\n\n\nDisclaimer: Contact details and evaluation responses from participants are collected and shared with the contributing PHNs to provide presentation materials\, invite feedback\, and deliver post-event support. Deidentified attendance details and evaluation responses will be shared with the Australian Digital Health Agency. \n\n\n\nAudience \n\n\n\nHCPS\, Primary Care Health Care and Aged Care workers. \n\n\n\nThis event is in partnership with Victorian-Tasmanian Primary Health Network Alliance (VTPHNA)
