DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nMentoring is defined as a relationship between an experienced and a less experienced person in which the mentor provides guidance\, advice\, support and feedback to the mentee to support their future experience and growth. \n\n\n\nThis workshop is designed for palliative care health professionals working in PEPA/IPEPA  Host Sites who wish to become a PEPA/IPEPA  mentor\, and share their palliative care knowledge with others and further develop their mentoring skills. \n\n\n\nFollowing the workshop\, participants should be able to identify principles for culturally safe\, supportive mentoring in the context of palliative care. \n\n\n\nFor those who may have been PEPA/IPEPA Mentors in the past (Prior to July 1st 2022) this workshop will act as a refresher program to ensure your ongoing professional development as a PEPA/IPEPA Mentor. \n\n\n\nAll those wishing to act as a PEPA Host Site Mentor for their workplace in Victoria\, need to complete the online Mentor module and attend one of the regularly scheduled PEPA Mentor training workshops in order to do so\, unless they can provide evidence of completing a substantive training program in Mentoring or education that can be considered equivalent. \n\n\n\nThis is essential to ensure that participants in PEPA/IPEPA placements have support and resources from our Host Sites to get the best learning outcomes possible. PEPA Victoria is committed to ensuring ongoing professional development and learning opportunities for Host Site Mentors within our quality framework. These include: \n\n\n\n1. Completion of pre-recorded video modules: Between each webinar\, video lectures (approx. 30 mins) will be available for participants to watch at time that suits them. The interactive webinar activities will be based on learnings from these video lectures. \n\n\n\n2. Live webinars: Attendance of three webinars is compulsory for successful completion of the course and is required to receive accreditation. SUMMARY:Improving Childhood Asthma Management (ICAM) Community of Practice - Back to School 2025
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nThis community of practice (CoP) session will focus on asthma action plans (AAPs)\, communicating with schools and managing asthma in children and adolescents in a school based setting. There will be case studies included in this CoP. \n\n\n\nCommunity of practice (CoP) sessions aim to improve clinician confidence in managing childhood asthma\, and foster collaboration between asthma care services in the inner west of Melbourne\, the Royal Children’s Hospital (RCH)\, Asthma Australia and the National Asthma Council Australia. Discussions and findings can also be applied to the wider childhood asthma community.  \n\n\n\nThe CoP provides an opportunity for all asthma care providers managing children to come together to improve their clinical knowledge and confidence. The sessions build the confidence of clinicians to access resources and services to better link patients into the system.  \n\n\n\nParticipants are multidisciplinary clinicians from community\, primary and tertiary care services. Members play key roles identifying current barriers\, and work together to develop local solutions to improve the care and service delivery to children living with asthma.  \n\n\n\nOpportunity will be provided for members to bring forth complex\, de-identified case studies to be discussed within the group. This allows members to learn from a multidisciplinary team and better understand the role different care providers have as part of the asthma care team. When available\, local data will also be shared to better understand how the system is performing\, including sharing of hospital emergency admission rates of children with asthma. Clinical content will be covered by subject matter experts\, including specialists from RCH.  \n\n\n\nIf you would like to submit a deidentified case for discussion\, suggest other topics for upcoming sessions\, or join the ICAM CoP Basecamp\, please email education@nwmphn.org.au  \n\n\n\nFacilitators  \n\n\n\nDr Katherine Chen is a paediatrician and the clinical lead of the short stay unit at RCH. Katherine graduated from the University of Melbourne in 2002 and completed her PhD in 2018. In addition to her clinical expertise in acute paediatrics\, she is a clinician scientist fellow in the Murdoch Children’s Research Institute’s health services and economics group. Her research interest is improving the care of children with asthma and wheeze.  \n\n\n\nDr Kirsty Tamis works as a GP in Truganina in Melbourne’s west. She is a member of the Royal Australian College of General Practitioners\, Royal College of General Practitioners and Royal College of Physicians in the UK. She has completed her Diploma of Child Health and is passionate about supporting parents and children towards better holistic health and living.  \n\n\n\nDr Shivanthan Shanthikumar is a paediatric respiratory specialist and clinician-scientist at RCH. SUMMARY:Syphilis: Advanced Webinar for Primary Care
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nVictoria has seen a 476% increase in Syphilis cases since 2010\, with a record 1671 cases reported in 2019. Of strong concern is the re-emergence of congenital syphilis since 2017\, despite no cases reported between 2004 and 2016. The outer north western suburbs of Melbourne have emerged as a hotspot for infections in heterosexuals of reproductive age. Hosted by the Victorian HIV and Hepatitis Integrated Training and Learning program\, this course builds on VHHITAL’s STI Essentials course and provides the skills and resources required for primary health care practitioners to confidently implement guideline-based management of patients diagnosed with syphilis and to improve patient outcomes. By embedding a framework of routine assessments to identify increasing palliative and end-of-life needs\, PACOP guides care planning to address these needs using an evidence-based practice and evidence-based responses. \n\n\n\nPACOP has developed a model of care that routinely monitors all residents\, thereby assisting in the early identification of palliative and/or end of life care needs. The model has two components which make up the complete PACOP program: \n\n\n\n1. The Profile Collection – routine\, regular\, sustainable assessments that monitor for any baseline changes in the resident \n\n\n\n2. The Outcomes Collection – residents identified with palliative and/or end of life care needs\, are transitioned to this part of the program where closer monitoring of the residents means rapid response to distress\, symptom management and care planning \n\n\n\nThis workshop provides an overview of the foundational knowledge required to implement the PACOP Profile & Outcomes Collection within your ACH. \n\n\n\nRegistration open from 8:30am for a 9am sharp start. \n\n\n\nOther information \n\n\n\nPACOP is a voluntary program funded by the Australian Government Department of Health and Aged Care and is available without cost to all ACHs throughout Australia. \n\n\n\nPACOP utilises a Train the Trainer approach to promote ongoing support and sustainability of the program within your ACH. DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nThis free online training aims to provide participants with the knowledge and confidence to increase the management of HCV in alcohol and other drug settings. \n\n\n\nPeople who inject drugs and experience social and structural inequities such as homelessness are at increased risk of acquiring HCV and are therefore a priority population for HCV models of care. \n\n\n\nHosted by the Victorian HIV and Hepatitis Integrated Training and Learning program\, this Hepatitis C Community of Practice session is intended to support the alcohol and other drugs workforce with the opportunity to bring case based studies for discussion and mutual learning. This peer learning approach enhances the practice of AOD workers to recognise those who may be living with or at risk of HCV and supporting them onto the care pathway\, particularly if they are disengaged with traditional health care services. Linking attendees with expert knowledge in point of care testing\, prison settings\, nurse led models of care and community facing support services\, the Community of Practice aims to support the elimination of Hepatitis C in Victoria. \n\n\n\nAll practitioners are invited to submit cases for discussion. SUMMARY:Workshop 1: St Vincent's Hospital Melbourne (SVHM) CareSynergy - Barriers to Discharge from Specialist Clinics
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nSt Vincent’s Hospital Melbourne (SVHM) invites GPs to attend 2 workshops (25th March and 27th May 2025) to explore barriers to discharge from specialist clinics and associated interventions to overcome these barriers. \n\n\n\nIn 2024\, they developed resources to facilitate ‘Supported Discharge’ from specialist clinics for patients with chronic Hep B (not on treatment) and those who had undergone bariatric surgery. These resources drew on established guidelines and input from our gastroenterology and bariatric surgery teams to develop handover tools. \n\n\n\nThis work highlighted the broad range of barriers to discharge\, and served as a prompt for St Vincent’s current project ‘Supported Discharge from Specialist Clinics’: A Study of Feasibility and Anticipated Value\, which is funded by Eastern Melbourne PHN’s CareSynergy Program.
