DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nAs part of Dementia Action Week 2025\, the South East Public Health Unit (SEPHU)\, in collaboration with the South Eastern Melbourne Primary Health Network (SEMPHN)\, invites primary care professionals to a vital webinar focused on reducing falls in people living with dementia.  \n\n\n\nFalls are the leading cause of injury-related hospitalisations and deaths in Australia. For people living with dementia\, the risk is even more pronounced: \n\n\n\n\nThey are at least twice as likely to fall as those without dementia.\n\n\n\nThey are 2 to 3 times more likely to experience adverse events such as further falls and delirium during hospitalisation.\n\n\n\nFollowing a first hospitalised fall\, about 2 in 5 people living with dementia in the community enter residential aged care.\n\n\n\n9 in 10 people living with dementia in residential aged care who are hospitalised due to a fall sustain an injury.\n\n\n\n\nThese statistics highlight the urgent need for targeted prevention strategies—especially in primary care settings where early intervention can make a significant difference. \n\n\n\nWho should attend?\n\n\n\nThis session is for nurses\, general practice staff\, allied health or anyone working in primary care.

The session will explain how to detect and manage breaches\, including immediate actions\, documentation\, and communication protocols to ensure vaccine safety and compliance. \n\n\n\nThe session also outlines best practice guidelines for vaccine storage\, continuous temperature monitoring\, and reporting requirements.  \n\n\n\nThis is ideal for all general practice staff who play a part in cold chain practices\, particularly those responsible for management. Attendees will leave with practical knowledge to ensure vaccine cold chain practices are safe\, compliant\, and accreditation-ready. \n\n\n\nSpeakers: \n\n\n\nSara Drew is a seasoned registered nurse and nurse immuniser with a rich background in primary health care\, public health\, immunisation\, chronic disease\, management and education. She is dedicated to supporting and strengthening the role of primary health care nurses through workforce development and education. Sara holds a Grad Cert in Infection Prevention and Control through Griffith University and is a member of the RACGP Technical Working Group -infection prevention and control\, participating in the most recent review of the IPC guidelines for general practices and other office-based and community-based practices. \n\n\n\nDeborah Kerz is a highly experienced registered nurse and diabetes educator with over 30 years of service across public and private health sectors. Currently serving as a Nurse Education Officer at the Australian Primary Healthcare Nurses Association (APNA)\, she brings a wealth of knowledge in acute care and over 12 years of dedicated experience in primary health care within general practice. She holds qualifications in diabetes education and management and a Certificate IV in Training and Assessment.
