SUMMARY:Workshop 1: St Vincent's Hospital Melbourne (SVHM) CareSynergy - Barriers to Discharge from Specialist Clinics
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nSt Vincent’s Hospital Melbourne (SVHM) invites GPs to attend 2 workshops (25th March and 27th May 2025) to explore barriers to discharge from specialist clinics and associated interventions to overcome these barriers. \n\n\n\nIn 2024\, they developed resources to facilitate ‘Supported Discharge’ from specialist clinics for patients with chronic Hep B (not on treatment) and those who had undergone bariatric surgery. These resources drew on established guidelines and input from our gastroenterology and bariatric surgery teams to develop handover tools. \n\n\n\nThis work highlighted the broad range of barriers to discharge\, and served as a prompt for St Vincent’s current project ‘Supported Discharge from Specialist Clinics’: A Study of Feasibility and Anticipated Value\, which is funded by Eastern Melbourne PHN’s CareSynergy Program. CareSynergy is aimed at improving integration between hospitals and primary care across the North East Metro. \n\n\n\nThe SVHM CareSynergy project is undertaking stakeholder consultation (including GP workshops) and other research to identify \n\n\n\n– barriers to discharge from clinics\, and \n\n\n\n– resources/interventions/relationships which overcome these barriers \n\n\n\nWe will then develop a model(s) of care which incorporates these adaptations. \n\n\n\nThe project seeks to promote a sense of collaboration between hospitals and primary care\, contribute to the efficient utilisation of specialist clinics and optimise access for additional patients requiring non-GP specialist assessment/care. \n\n\n\nWorkshop Details: \n\n\n\n\nWorkshop 1 – identifying barriers and proposing solutions\n\n8:30 am – 11 am\, Tuesday 25th of March 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus\n\n\n\n\n\nWorkshop 2 – review of proposed care model(s)\n\n8:30 am – 10:30\, Tuesday 27th of May 2025\n\n\n\nSt Vincent’s Hospital Melbourne\, Fitzroy Campus OR via Microsoft Teams meeting (to be decided)\n\n\n\n\n\n\nYou will be remunerated for your time at a rate of $150/hour (+GST) including 0.5 hours of pre-reading. \n\n\n\nIf you are interested and available to attend\, please reply via rachael.sutherland@svha.org.au or to joint project leads Cath White catherine.white@svha.org.au and Meg Marmo meg.marmo@svha.org.au. for this event.

In this session\, we will hear from the Department of Fairness\, Families and Housing (DFFH) about their processes managing information sharing requests. We will also have a Q&A with a panel of experts including representatives from Anglicare Victoria (AV) and 54 reasons (formerly Save the Children). \n\n\n\nWhat are the outcomes of the session?• Develop a better understanding of when to share information using CISS or make a mandatory report to Child Protection• Learn how CISS can support child wellbeing and safety when there is a shared understanding of CISS across services/workforces• Get a behind the scenes insight into how DFFH manages information sharing requests• Hear from our panel of experts on how they use CISS in a multi-service settingWho should attend?This session is relevant for Victorian primary health care nurses and members of the multidisciplinary health care team working with children and families.APNA’s Improving Child Wellbeing and Safety Outcomes project is supported by the Victorian Government under the Child Information Sharing Capacity Building Grants Program.
