DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nIn this session\, attendees will be introduced to a Quality Improvement (QI) activity using the Hospital Avoidance Tool (HAT) report in Polar to identify patients at urgent risk of hospitalisation and increase your number of completed General Practitioner Management Plan (GPMP) or review of GPMP for eligible patients.  \n\n\n\nThis QI activity will also support continuity of care activities for identified patients by offering MyMedicare registration and uploading Shared Health Summaries. \n\n\n\nLearning Outcomes: \n\n\n\nAt the end of the session\, participants will be able to: \n\n\n\n\nUpdate your team on hospitalisation risk\, GP Management Plan (GPMP) or review of GPMP\, MyMedicare and Shared Health Summaries.\n\n\n\nIdentify patients at urgent risk of hospitalisation using the Hospital Avoidance Tool (HAT) report in Polar.\n\n\n\nImplement activities to increase number of completed GP Management Plan (GPMP) or review of GPMP for identified patients at urgent risk of hospitalisation.\n\n\n\nOffer MyMedicare registration and Shared Health Summary upload for those identified patients at urgent risk of hospitalisation.\n\n\n\n\nAttendees will be provided with the Identifying patients at risk of Hospitalisation Quality Improvement Toolkit\, POLAR walkthroughs and Plan-Do-Study-Act examples.

The presentation will be delivered by Dr Aaron Wong\, the clinical director of the palliative care service at Eastern Health. Dr. Wong is also a dual trained geriatrician and palliative care physician. \n\n\n\nSession summary:                                                                                                         In Australia\, more than half of permanent residents in aged care facilities are living with dementia. A significant proportion of these residents have unmet palliative care needs and often experience inadequate pain control\, fewer palliative care intervention and increased hospital admissions which may be burdensome and distressing. These facilities face multifaceted challenges in managing advanced dementia\, including limitations in staffing capacity and confidence\, complexity in clinical need\, care fragmentation and barriers to accessing palliative care. Effective and timely palliative care is essential in avoiding unwanted and distressing hospital admissions\, supporting preference-based care and improving quality of life. Given that GPs provide the majority of the medical care for this large and increasing group of people\, GP education and support for delivery of palliative care is essential in reducing this unmet need.
