This practice-wide quality improvement exercise will enable you to identify potential barriers to vaccine uptake and check your performance against evidence-based benchmarks. \n\n\n\nThere are two pathways for this program\, one for GPs\, and one for Practice staff (including practice managers\, practice nurses and other relevant practice staff). Please ensure you select the right program for your position. \n\n\n\nThis education is designed to take place over multiple weeks. It does not need to be completed in one sitting. You can stop at any time\, and progress will be saved. The sessions will introduce the plan\, discuss practical implementation\, and provide information on participating in the current project trial. \n\n\n\nAboriginal Health Workers\, community organisations\, hospitals\, aged care homes\, and general practices in Victoria are encouraged to attend. \n\n\n\nThe sessions for the Easten Melbourne PHN are below (please note you can select any date when registering) \n\n\n\n\nTuesday 8th October 1pm to 2pm\n\n\n\nTuesday 13th October 6:30pm to 7:30pm

DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nThis 45-minute session is aimed to introduce a current project that Palliative Care Victoria is leading in First Nations Aboriginal Palliative Care. \n\n\n\nPalliative Care Victoria received funding from the Commonwealth and State government to develop a culturally-safe trauma-informed palliative care plan in partnership with Aboriginal and Torres Strait Islanders people to support Elders living in Residential Aged Care Home (RACHs). \n\n\n\nIn collaboration with our community partners\, we have developed the ‘Yimgawugel bungim yiya’ -Songlines for Deep Dreaming: Let’s Yarn Cultural Considerations for Palliative Care Plan that supports Aboriginal and Torres Strait Islanders patients and their families to document their cultural care considerations while they access culturally-safe and effective palliative and end of life care. \n\n\n\nThe implementation of this document will expand on existing models of care while supporting new approaches to the way care is delivered or commissioned to our First Nations Elders within Victoria. \n\n\n\nThis session will introduce you to this document and provide an opportunity in how your practice can implement the plan and be part of the current project trial.

She continues to work at the busy Joan Kirner Women’s and Children’s at Western Health\, providing gynaecological\, pregnancy and birth care to women in Melbourne’s west. Althea has held positions with RANZCOG and the University of Melbourne providing teaching and mentorship to junior doctors and medical students. Althea is passionate about providing care that is kind\, supportive\, and evidence-based\, so that individuals and families can make informed decisions about their care. \n\n\n\nDr Iniyaval Thevathasan is an obstetrician and maternal fetal medicine subspecialist with a private practice at Melbourne Maternal Fetal Medicine in Fitzroy and a public appointment at The Royal Women’s Hospital. She provides comprehensive pregnancy care and has particular interests in the prevention of preterm birth\, the management of previous adverse pregnancy outcomes\, multiple pregnancies\, and the care of women with inflammatory bowel disease during pregnancy. \n\n\n\nLearning outcomes:\n\n\n\nShared decision making and influences on birth choices \n\n\n\n\nDiscuss maternal and family decision-making during pregnancy and birth\, and evaluate the benefits\, risks and considerations of different planned modes of birth.\n\n\n\nApply current evidence-based approaches to Group B streptococcus (GBS) screening and management in the third trimester.\n\n\n\n\nIncidental findings on pregnancy ultrasound: what to look for\, when to be concerned and when not to \n\n\n\n\nExplain the clinical significance of incidental and significant ultrasound findings in pregnancy\, including their implications for clinical management.\n\n\n\nDescribe the management\, support services and referral pathways available for women experiencing significant ultrasound findings in pregnancy.\n\n\n\n\nBe sure to register for both workshops \n\n\n\nSession 2: Shared maternity care\, workshop 2. \n\n\n\nBy registering for this workshop\, you acknowledge and consent to your personal details being shared with the participating hospitals for the purposes of program delivery and administration
