This practice-wide quality improvement exercise will enable you to identify potential barriers to vaccine uptake and check your performance against evidence-based benchmarks. \n\n\n\nThere are two pathways for this program\, one for GPs\, and one for Practice staff (including practice managers\, practice nurses and other relevant practice staff). Please ensure you select the right program for your position. \n\n\n\nThis education is designed to take place over multiple weeks. It does not need to be completed in one sitting. You can stop at any time\, and progress will be saved. You can find education you have started on your Dashboard. \n\n\n\nOn completion of this quality improvement program\, you will be able to:  \n\n\n\n\nUnderstand the current status of COVID-19 in Australia\, in particular its impact on older patients in primary care \n\n\n\nClarify the role of vaccination in preventing COVID-19-related morbidity and mortality in older patients \n\n\n\nReview current best practice recommendations for COVID-19 vaccination in patients aged 65 and older \n\n\n\nUsing a team-based approach\, decide on quality improvement targets and actions related to improving uptake of COVID-19 vaccines in older patients in primary care \n\n\n\nImplement and reflect on practice-wide quality improvement initiatives for increasing COVID-19 vaccine uptake

DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nUsing the experiences of children with medical complexity and their families\, this webinar will explore how trust develops over time through continuity\, relationships and repeated interactions with services.  \n\n\n\nIt will provide practical guidance on developing trust companions: consistent people who help connect care across services and provide continuity throughout a person’s care journey.  \n\n\n\nThe webinar will examine how trust can be supported through roles\, team practices\, service design and wider system change.  \n\n\n\nIt will also consider what services can do when trust has been damaged and how they can create the conditions for trust to be rebuilt.  \n\n\n\nParticipants will explore how to: assign clear responsibility for building and maintaining trust; create continuity across teams\, services and transitions; design roles\, processes and models of care that support trusting relationships; identify and change system practices that undermine trust\, including how trust can be repaired when it is lost.
