This practice-wide quality improvement exercise will enable you to identify potential barriers to vaccine uptake and check your performance against evidence-based benchmarks. \n\n\n\nThere are two pathways for this program\, one for GPs\, and one for Practice staff (including practice managers\, practice nurses and other relevant practice staff). Please ensure you select the right program for your position. \n\n\n\nThis education is designed to take place over multiple weeks. It does not need to be completed in one sitting. You can stop at any time\, and progress will be saved. Each session is designed to support shared learning\, strengthen local connections and explore practical approaches to improving coordination of care for residents. \n\n\n\nWho should attend\n\n\n\nThis session is suitable for: \n\n\n\n\nGeneral practice staff\, particularly GPs providing care to residents in RACHs\n\n\n\nPractice nurses and nurse practitioners supporting aged care work\n\n\n\nRACH staff\, including clinical managers\, facility managers\, and relevant corporate roles\n\n\n\nOther healthcare providers involved in delivering care to residents in RACHs\n\n\n\n\nWhat to expect\n\n\n\nEach quarter will have a different theme.
