SUMMARY:Improving Childhood Asthma Management (ICAM) Community of Practice - Back to School 2025
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nThis community of practice (CoP) session will focus on asthma action plans (AAPs)\, communicating with schools and managing asthma in children and adolescents in a school based setting. There will be case studies included in this CoP. \n\n\n\nCommunity of practice (CoP) sessions aim to improve clinician confidence in managing childhood asthma\, and foster collaboration between asthma care services in the inner west of Melbourne\, the Royal Children’s Hospital (RCH)\, Asthma Australia and the National Asthma Council Australia. Discussions and findings can also be applied to the wider childhood asthma community.  \n\n\n\nThe CoP provides an opportunity for all asthma care providers managing children to come together to improve their clinical knowledge and confidence. The sessions build the confidence of clinicians to access resources and services to better link patients into the system.  \n\n\n\nParticipants are multidisciplinary clinicians from community\, primary and tertiary care services. Members play key roles identifying current barriers\, and work together to develop local solutions to improve the care and service delivery to children living with asthma.  \n\n\n\nOpportunity will be provided for members to bring forth complex\, de-identified case studies to be discussed within the group. This allows members to learn from a multidisciplinary team and better understand the role different care providers have as part of the asthma care team. When available\, local data will also be shared to better understand how the system is performing\, including sharing of hospital emergency admission rates of children with asthma. Clinical content will be covered by subject matter experts\, including specialists from RCH.  \n\n\n\nIf you would like to submit a deidentified case for discussion\, suggest other topics for upcoming sessions\, or join the ICAM CoP Basecamp\, please email education@nwmphn.org.au  \n\n\n\nFacilitators  \n\n\n\nDr Katherine Chen is a paediatrician and the clinical lead of the short stay unit at RCH. Katherine graduated from the University of Melbourne in 2002 and completed her PhD in 2018. In addition to her clinical expertise in acute paediatrics\, she is a clinician scientist fellow in the Murdoch Children’s Research Institute’s health services and economics group. Her research interest is improving the care of children with asthma and wheeze.  \n\n\n\nDr Kirsty Tamis works as a GP in Truganina in Melbourne’s west. She is a member of the Royal Australian College of General Practitioners\, Royal College of General Practitioners and Royal College of Physicians in the UK. She has completed her Diploma of Child Health and is passionate about supporting parents and children towards better holistic health and living.  \n\n\n\nDr Shivanthan Shanthikumar is a paediatric respiratory specialist and clinician-scientist at RCH. He works in the complex asthma clinic\, and was recently involved in helping to put the new asthma clinical practice guidelines together. \n\n\n\nSubscribe here to Asthma Australia’s Asthma Link – connecting healthcare professionals with news & opinion across the asthma community. \n\n\n\nLearning outcomes:\n\n\n\nBy the end of this activity\, participants will be able to: \n\n\n\n\nInterpret local data and identify potential solutions to improve asthma care locally. \n\n\n\nImplement best practice management for asthma in children\n\n\n\nDescribe resources and local services available for children living with asthma\n\n\n\nIdentify collaborative\, multidisciplinary opportunities to improve care for children living with asthma

SUMMARY:Community of Practice for Palliative Care in General Practice
DESCRIPTION:Date\n\n\n\n\n\n\n\n\n\nVenue\n\n\n\n\n\n\n\n\n\nOrganiser\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nJoin Our Community of Practice for Palliative Care in General Practice! \n\n\n\nThe Eastern Melbourne Primary Health Network (EMPHN) is excited to invite passionate general practitioners (GPs) and GPs in training to join our Community of Practice for Palliative Care in General Practice. \n\n\n\nPartnering with the Eastern Metropolitan Region Palliative Care Consortium (EMRPCC) and Eastern Palliative Care\, this initiative aims to elevate best practices and enhance palliative care capabilities within our community. \n\n\n\nThis session will focus on palliative care in RACH\, including role of palliative care for those living with dementia. The presentation will be delivered by Dr Aaron Wong\, the clinical director of the palliative care service at Eastern Health. Dr. Wong is also a dual trained geriatrician and palliative care physician. \n\n\n\nSession summary: In Australia\, more than half of permanent residents in aged care facilities are living with dementia. A significant proportion of these residents have unmet palliative care needs and often experience inadequate pain control\, fewer palliative care intervention and increased hospital admissions which may be burdensome and distressing. These facilities face multifaceted challenges in managing advanced dementia\, including limitations in staffing capacity and confidence\, complexity in clinical need\, care fragmentation and barriers to accessing palliative care. Effective and timely palliative care is essential in avoiding unwanted and distressing hospital admissions\, supporting preference-based care and improving quality of life. Given that GPs provide the majority of the medical care for this large and increasing group of people\, GP education and support for delivery of palliative care is essential in reducing this unmet need. This presentation will explore the principles\, challenges and support options including novel models of care for optimising the delivery of palliative care for residents living with dementia. \n\n\n\nGet Involved:Don’t miss this opportunity to be part of a dynamic and supportive community focused on improving palliative care. Join us in making a meaningful impact in palliative care within our region. Please note this is a RACGP CPD accredited activity. \n\n\n\nExpress your interest via this link here.
