Join Our Community of Practice for Palliative Care in General Practice!

The Eastern Melbourne Primary Health Network (EMPHN) is excited to invite passionate general practitioners (GPs) and GPs in training to join our Community of Practice for Palliative Care in General Practice.

Partnering with the Eastern Metropolitan Region Palliative Care Consortium (EMRPCC) and Eastern Palliative Care, this initiative aims to elevate best practices and enhance palliative care capabilities within our community.

The evening will feature two insightful presentations. The first, titled ‘Phone a Friend – Where to Get Help from Specialist Palliative Care,’ will be presented by Dr. Katie Tham from Eastern Palliative Care and Esther McMillan-Drendel from the Palliative Care Advice Service. This presentation will explore the specialist palliative care services available in the region and how to access help. This will include information about the valuable support offered by the Palliative Care Advice Service, who assist those living with a life-limiting illness, providing crucial information for families, carers, and the community, to navigate the palliative care health system and find suitable support and care.  

The second presentation, ‘Minimising Barriers to Timely Access of Medical Care for Patients and Their Families Living at Home with Death and Dying’, will be presented by Dr Katie Tham and Dr Chien Lin from Eastern Palliative Care (EPC). It is known that many patients and their families prefer to maximise the time in their places of residence for patients living with dying. As patients are frequently living with intensifying disability inherent to dying, domiciliary availability of care is fundamental to minimise the need of hospital attendances for patients and their families during precious survival time. While EPC clinicians specialise in working in the community, the Specialist General Practitioners (GP) specialise in long-term care of their patients and their families and remain the main medical care provider for all patients living with dying in their places of residence. This session will explore strategies to enhance communication between healthcare providers and patients, streamline referral processes, and minimise barriers to deliver home-based care services

Our Community of Practice is designed to:

  • Promote Best Practices: Share and implement the latest best practices in palliative care.
  • Build Capability and Capacity: Strengthen the community’s ability to provide highquality palliative care.
  • Enhance Professional Development: Offer ongoing support and professional growth opportunities for GPs.

Meeting Frequency & Location:

  • Regular Meetings: One hour every two months, a combination between online and inperson gatherings.
  • Meeting Format: To be tailored based on member feedback during our inaugural meeting.
